Remove the card from its top loader and penny sleeve, and wipe the front and back with a microfiber cloth. Place it on a clean surface that's a different color from the card's borders (a colored surface shows edge and corner wear better than white). Photograph the front and back straight-on with no shadows or glare, then tap Scan Card.
